COVID-19 Form Templates
In the midst of the Covid-19 pandemic, access to clear and credible data is more important than ever. Data is not only informing policy decisions and public health initiatives, but during this unusual time the *general public* is also seeking out data to inform how to feel about where we stand in our efforts to combat Covid-19.
While outbreaks move quickly through communities, it's sometimes hard to keep up. That's why we're designing and open-sourcing data collection forms that your teams can use in their efforts to stop the spread and provide data to public health workers, policy makers and your community.
Currently two forms are available for download on our GitHub:
These forms can be downloaded and used on any mobile data collection platform that accepts XLSForms, including Secure Data Kit which supports offline use in the most resource constrained areas, and is already used in over 60 countries around the world for the purpose of tracking the spread of disease. Our team is ready and on standby to customize these forms to better serve the specific requirements of your team, and we are currently offering 24-48 hour turnaround times (without additional rush fees) so that you can start gathering quality data as quickly as possible.

- Confirmed Cases & Close Contacts — Use this form to assist in your contact tracing efforts. Log confirmed cases and all contacts they've been within 2 meters of in the past 2 weeks.
